Dr Kaleb Lourensz ("we") is committed to protecting your personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.

What We Collect

When you use this website, we collect the information you choose to provide through our appointment request form: your name, phone number, email address, the service your enquiry is about, your preferred location, and your message.

When you become a patient, we collect health information needed to provide your care, including from you, your referring doctor and other healthcare providers involved in your care.

How We Use It

To respond to your enquiry, arrange appointments, provide and coordinate your care, communicate with your referring doctor and other treating practitioners, and meet our legal obligations, including billing Medicare and your health fund.

Disclosure

We disclose your information only to people involved in your care, such as your GP, other specialists, hospitals, pathology and imaging providers, and the members of our care team, or where required or permitted by law. Form submissions are processed by our website provider, Squarespace, whose servers may be located overseas.

Website Analytics and Cookies

This website uses Squarespace's built-in analytics and cookies to understand how the site is used. We do not use advertising trackers.

Security

We take reasonable steps to protect your information from misuse, loss and unauthorised access.

Access and Correction

You may request access to, or correction of, the personal information we hold about you by contacting the rooms.
